I took your advice and called my insurance company and asked them straight out if 100mg of enbrel/week is covered.  They looked it up, and yep, it is covered but, of course, requires pre-authorization.  I have a strong feeling, though, that what they told me is dx dependent, meaning the 100 mg/week is covered for psoriatic arthritis because that dosage has been approved for that dx. But it has not been approved for rheumatoid arthritis.  In any case, I have my next injection tomorrow.  If I feel a clear benefit from it, I'll call my rheumatologist on Monday and inquire further what the issue is with the extra dose.
Current dx: Rheumatoid Arthritis
Suspected dx: Early Lupus/MCTD; Inflammatory Bowel Disease
Current Meds: Enbrel, Plaquenil, Aciphex, Ultracet, Zyrtec, Allavert-D, Zantac, Tylenol PM
Past Meds: Relafen, Vioxx, Mobic, Voltaren, Sulfasalazine, Entocort, Prednisone, Humira, Reglan
