How to wean off fentanyl patch

Any suggestions for how to wean oneself off of a fentanyl patch? I have the lowest dosage, and need some help with pain, but the whole patch feels like too much. I also have oxycodone pills, but would like to limit my use of them since they are harder on the organs. I had major abdominal surgery about 9 weeks ago for colon cancer. Still have cancer in my liver and some in my pelvis. Thanks.

Welcome to the chronic pain group. I'm glad you found us but sorry that pain brings you here. You need to be in a doctor's care to help and direct you on weaning off the patch. I'm on the patch, but I don't plain to go off of it. I'm afraid none of us can direct you on weaning of the patch. We're not doctors, but we all know about chronic pain.

I wish you good luck if that is what you wish to do. With you just having surgery 5 weeks ago, I would think that you would want all the help you can get with the pain, especially since you have cancer in your liver and elsewhere.

Dx: osteoarthritis, bursitis in left hip, Osteoarthrits in right hip, compression fracture in thoracic spine due to falling on frozen ground March 2001 , ddd, spinal stenosis, bone spurs, osteoarthritis in spine, osteoarthritis in both knees
Meds: Fentanyl patch, oxycodone, otc: BenGay, Tylenol Arthritis on occasion

is the patch harder to stop than oxycodone pills? i know they are totally different but my doc is thinking that he wants me to try the patch as soon as it cools off a bit here in AZ...are they a lot stronger? more narcotic effects? right now i'm taking 15 mg oxycodone x 6 for pain associated with crohn's disease...problem is the pills don't last long with my body...my absorbtion of anything is horrible so with the pills 2 hrs after i take them i'm in pain again...i'm nervous about the patch though...hope you feel better soon Graymet and you kick that cancer's &^%!!!!!

mgmtman, I live in AZ with a patch...actually I think I had it on when we met up for lunch (which the next one, lunch/dinner, is on me k? whenever you feel like it)...the patches have changed in the last few years...they no longer have a gel packet, but are a small adhesive strip like a bandaid...and the reason why I'm on them is much the same reason as you (crohn's makes meds go through too quickly)...

Grayment, just wondering why you feel it's "too much"...I think they may have tried you on the patch for the reason mgmtman and I are on them...when the digestive system doesn't work right(ie, your adominal surgery), the pain meds don't either...they don't metabolize correctly...

I've been on the low dose of the patch for over 3 years now...haven't had to go up in dose (though some days I think I need to, other days I'm okay)...anyways, if you have questions, go ahead...I hope all goes well and take care...

really? good to know i've got a local expert! i'm ready to give it a shot- i was just scared sweating would send the meds into my system and then on the last day i'd be in trouble bc i'd have nothing left in the patch...do you only use the patch or do you upplement with anything? my doc was talking about keeping me with 3-4 hydrocodones a day also...talk to you soon!

I have meds for break through pain, and have meds for times when the crohn's kicks it up to "I'm ready to die now"...I used to have to go into ER at these times but since I'm on a lot of immunosuppresent drugs, my docs want me to stay away from the ER unless I'm really, like, you know, dying...
BTW, if you want to give me a call(I think you still have my number) or email me, I'll let you know the in and outs of the patch (ie, how it apply, when and were), or if you need just someone to talk too...it seems as if you are in rough patch right now...take care...

yeah definitely. i'll be in touch soon. things have been really rough. i've been going to work, coming home and basically trying not to fall apart in front of my daughter. i haven't ever felt this poor. none of the meds really seem to work so i think the knife it is! haha. oh well i guess i put it off for 20 years. i knew in the back of my mind it was coming sometime. hey how do you handle it if you run out of your meds before the end of the month? i guess with the patch that's less of a problem. my issue is that with the pills i always feel i'm playing catch up. everything goes right through me without absorbing so what seems like a lot of pills for me is not (for someone else without digestive issues it may be a lot). my docs have been super cool but it seems as if every month i'm totally freaking out for a few days or a week wondering if i'm going to make it to the next refill- -- horrible thing to deal with along with everything else. oh well- maybe you have some tips on that too. i'll give you a call this week and we'll get together- if not for a meal (I'm not really eating) maybe for an iced tea or something. talk to you soon!

Hello Graymet!

Welcome to our Chronic Pain Forum! I suppose if you are on the lowest dose, and wish to discontinue them, then you need to contact you're PM or PCD and see what they want you to do next.

If you don't mind me asking, what dose of Oxycodone are you taking for BT? If you are taking Percocet's then yes I understand the concern about the liver problems, because of the acetaminophen. However, if you are taking the Oxycodone IR then you should not have to worry as much about the organs.

I'm guessing you are still taking treatment for the cancer, and hopefully they can successfully treat it. We wish you the best, and feel free to visit us for support anytime.

Hello mtgman!

I'm wondering if you have thought about a liquid form of pain med's because of the absorption rate problem? Maybe it might help. Just a thought!

hi SE...no we've never discussed liquids. thanks for the suggestion...the patch was the only other thing we talked about besides the pills. i will mention this to my doc.

        I had a very good friend here on the forum that had a ileostomy, and suffered the same absorption problem you do. Pills did not work so well for her, and they switcherd her over to the liquid oxycodone, and it helped her greatly.
 
       Let us know what the DR says about it.
