Thrombosed Hemorrhoids

I know i have not been on much since my surgeries but I am still in pain and my GI will have me go for another MRIE if the pain is still evident at the anastamosis site in mid april since my Sx was january 11.  My joint pain and abd pain came back while I was waiting for the wound infection to heal and was able to begin Hunira again Feb 27th.  The last dose for me before that was Jan 4th.  My GI placed me on the normal 1 shot every other week post op ...pre op I was using 1 shot a week.  I saw my GI 3/17 and he noticed how tender the area was.  I also informed him that using the questran some days would seem to work others it wouldnt so he told me to stop.  I have severe pain when I take Iron pills but it doestn stop the D.  I figured I would have less troubles with hemmies with all the D that I have now as opposed to the C I had before my 4 surgeries.
 
Before this last surgery in Jan my surgeon looked at my anus after I was telling him of discomfort...I never had an external hemmie that had not bled before and when I did it barely bled and I would use the script beta val and it would be gone in a cpl days.   I informed him of the burning etc when I saw him on 3-11 and he gave me a sample of recta gel but told me to just use it topically and not as a suppository as it is meant for...the recta gel is a gel of lidocaine and cortizone and for you ladies who have used yeast infection cream you know the plunger I speak of that is what they give you to use this med as a suppository....I cannot imagine using a rectal suppository that isnt tapered at the insertion end.
 
I think I have new hemmies there I was putting some of the gel on topically and it felt like there were extra enlarge folds, in the mirror they are the same color as the surrounding tissue.  I also have a sensitive area a little further asway from the anus but still at least within half of an inch or so from the anus.  It seems tender to the touch when rubbing the gel on and seems like it may feel like vein beneath it.  I know what veins feel like that are really full of blood from my days as a vet tech and drawing blood from many diff veins on an animal...never near the anus obviously but enough to make me assume this is what I am feeling but with all of my complications I have had I fear that it could be a fistula forming.  I have only had 2 types of fistulas the 1st was intestine to intestine which was discovered only when I was opened for emergency Sx the second type was enterocutaneous at my belly button...well so close to it that my bellybutton now resides in a jar with the fistula affixed to one side of it in a jor with formalin in the pathology dept at the hospital I was in.  I began having abd pain in late february and was still recovering from a wound infection so I was unable to begin back on humira asap I began it but at the traditional dosing 1 every 15 days before Sx I was doing 1 shot each week and I noticed a big change in fistula output, size and also helping my joint pain.  I talked to my GI the 17th while there and we determined that I need to do the shot every weeek again which has helped my joints and my belly somewhat but still bad pain and it is the type of pain I had until the fistula came through and finally relieved pressure by comeing to the surface having the abscess pop followed by drainage of the feces
 
I am so scared that it will be another fistula either that or an abscess that is just festering.  I watch for fever etc and there is noe but I think I have a right to be afraid

If it's flesh colored it's NOT a hemmie...check out this link, the first set of pics is hemmies, the pics under those are perianal crohns skin tags which are flesh colored because hemmies are simply inflammed/inlarged veins, skin tags are just that tags of skin which is why they're flesh colored...

it is really hard for me to see what is really going on back there since where I live there is no mirror in a well lit place. I know before the surgeon said it was a thrombosed hemmie. Last night one bled a little as if it was a superficial fissure I may have struck but it was just spotting on the TP. If it is skin tags what is typically done? Do they hurt as well it hurts for me to pass gas, have a BM (and mine is still all D) and it hurts to sit at times too?
 
I too meant to add that it is itchy at times and I use the wipes and make sure that no fecal matter is left behind after a bm.  I will use a few of the wipes until I no longer see any poo left on them and I make sure to go around these swollen areas.
 
I am just so scared at this point I went 20 years with no Sx then bam I feel like the only one at times my husband feels badly and helps but at times he feels I am ok since I still alternate between good and bad days I too sometimes look okay but as we all know that is common to look okay but feel horrible.
 
What would these elephant ears mean as far as fistulas? active disease? I just need feedback to those who have the elephant ears and what docs have done to help with them, what meds have helped since it may mean that my CD remanifested again while I was waiting for the wound infection to go away before I was able to start back on Humira

Fistulas are not the same as "elephant ears" (anal skin tags) and just because you have anal skin tags does not mean you will have a fistula...I've never had a fistula and I've had the skin tags for over 18 yrs.

Sometimes they cause itch, but so do hemmies, maybe try a hand mirror in day light to have a look.

What do you do for your skin tags? Do yours tend to hurt even if you are just passing gas or passing stool? Is there anything you can put on them to help with the itching even when they are clean or help with the pain so it is not so painful when passing gas or stool?

I rub extra virgine olive oil on them, infact I use it all over my body as a moisturizer, even for my hair, it's very good for your skin and hair and has a natural pain stopping effect similar to pain meds....it'll help with the itching too.

You can also use licodane, it's a numbing gel and works great when the pain is really bad.

I know this probably sounds weird, but I had the same issue where I was like man why does my bum hurt so bad and I was wondering if there was anything visibly wrong with its exterior and I had no way to get a look at it.  I found that if you take a little mirror, like a makeup mirror or mini-mirror for your purse, and hold it with one hand, you can manage to get a pretty good look at any problem area, and if the lighting is poor you can use your other hand to shine a little flashlight at the mirror, which reflects onto your bum and at the proper angle also lets you see the lit-up area of your bum in the mirror.  LOL...I know that sounds bizarre but it worked pretty well.  I was disappointed to see there was nothing wrong with my bum, but then I got a colonoscopy a couple weeks later and found it was crohn's.

I had a thrombosed hermorroid once.  Initially I thought it was an abscess (painful bulge near the anus.)  My GI set me straight.  Said to to take long soaks in the bath and it cleared up in a week. This was years ago.  No recurrence.
