anyone ever have to fill out an adult function form?

  Quick question..... after having been denied once by SS, when I appealed, they decided to look harder at my case.  They have decided at this point that I  have been "approved before decison"  and they have now sent me a thick pack of papers called adult function forms.  I was wondering if anyone else has ever filled this out and should it be based on your worst days with Crohns ( and i suffer from lottsa side effects like mouth ulcers, PG  etc) or on the good days when u actually have those?  I dont want to lie but it seems like the questions are so black and white and they dont quite address all the gray areas.
Any advice would be appreciated.

Diagnosed 4 years ago. been  a helluva ride since.  If there is a side issue to this dd I can find it.  I suffer with Pyoderma Gangrosum and horriffic mouth ulcers that get to the point I can't open my jaws.  Last one was nice enough to create a fistula and  add to my list of fun things to deal with.
Been on all kinds a things, currently on Imuran 50 mgs and it at least give me some confidence in leaving my home.
Allergic to Remicade. Promethius test shows my body fights it.  Also suspected to have PBC or PSC  autoimmune liver disease.

Interesting. I recently won my battle with Disability after 1.5 years. It didn't take near as long as I thought. I got denied the 1st time and then my lawyer told me that this could take 3 years. Well 3 months later I won the battle after hiring the lawyer. I have never heard of those papers BUT fill them out ASAP honestly and with your worst days.... not the days that you act like you are okay (like most of us do). I hope someone replys to you with some answers and help w/that. Maybe someone on this forum- and I am sure someone has will have some answers for you!

I have had Crohn's since I was 21 years old and I am now 28!!
Unfortunately, nothing has worked for me. But I will keep on going and trying everything possible!

One of the things I always focus on when filling out forms regarding my disability is the unpredictability and fluctuation in the symptoms. You might want to discuss your functionality in terms of best, average, worst. For instance, on my best days I am in the washroom at least XX times. On my worst days I am in the washroom XX+ times. In the last two years the longest period I have gone between bad days is XXX. On my best days I am able to xxx. On my worst days I cannot leave the house etc...

30+ years living with Crohn's.

For mine the doctor who gave it to me instructed me to use worse case examples.
