Hello All!
I have reposted the earlier survey now that I have received prior permission from the Administrator/Founder Peter.
Here is the original post:
"I really need your help!  I am a graduate student with Crohn's Disease (27 years) and am doing my Master's Thesis on Self-Efficacy(confidence in completing a task) and those afflicted with Inflammatory Bowel Disease. 
I need 50 people to take a few minutes to fill out my survey online at the link below. 
IMPORTANT!!!  You must be 18 years of age or older and have Crohn's/Ulcerative Colitis/IBS.
The survey is completely ANONYMOUS. 
It is my hope that the results of this will help shed light on this very important factor in coping with these diseases and our ability to manage it.

I'd like to comment on the survey. I think the answers are very dependent on how a person's health is at the time. I am currently feeling pretty bad about my situation at the moment. I would answer very different when I am not doing so poorly. I would have liked to have been able to comment on my current health at the time of completing the survey, because otherwise I feel it does not reflect my usually situation when I am in remission. I don't know how others feel about this.
44 years old. Diagnosed with moderate to severe Crohn's in April 2005. Hiatus hernia diagnosed in 2008. Had Crohn's under control until March this year when I had a major flare up and ended up in hospital. Diagnosis is now CD and IBS. Getting it under control again.
Currently taking: Prednisone 12.5 mg, Salofalk 2000 mg, Nexium 40 mg, Calcium and Vitamin D, Matamucil, Yogurt for probiotics, Salofalk suppositories as needed.

I agree with WriterMum. I answered as how I would feel during a flare, not right now when I'm in remission.

My only comment would be that alot of the questions seemed to be repeats. Good luck with your thesis.
Crohn's Disease. 
Diagnosed 13+ years ago. 
Been in remission for short periods of time but each time I come out of remission my disease seems to have spread.  No longer isolated in my ileum, now from stomach down to rectum.  Currently have C-Diff.
Current Medications for the Crohns:  Humera 40mg injections every other week, Imuran 150mg, 3xday, Hyomax, Welchol 625mg, Flagyl 250mg, 3xday, Calcium, Acidophilus & Potassium
Previous Medications I've been on:  Asocal, Pentasa, Cipro, Prednisone & Entocort
