What should I expect/ask at my first GI appt in 6 years?

I'm seeing my GI tomorrow. A little background: was dx by a different GI after a scope and biopsy. Classic crohn's in the ileocecal valve and beginning of my colon. I didn't like the GI much so my PCP treated me during my flare (I also had no insurance and he hooked me up with the asacol drug rep so I just kept going to him). After 6 months, I was in remission, off all meds. I was still having some lingering issues so I called a new GI. He did a scope and saw no scarring, no inflammation, nothing. Since he didn't do my first scope, he couldn't confirm I had Crohn's, and just said I was currently dealing with IBS. He had doubts as to whether I ever actually had Crohn's, since there was no scarring (though I'm not sure how he can explain the test results from the first). This was 6 years ago and I've dealt with the occasional diarrhea (maybe 1-2x/week) and felt fine.

Twelve days ago, I believe I started flaring. I've had no solid BMs in 12 days, and diarrhea 2-6x/day. My WBC is slightly elevated (11), sed rate is normal (11), my hair is falling out (not clumps, just handfuls whenever I run my hands through it), I'm nauseous every time I eat. I have cramping shortly after eating. Tomorrow, I'm going to that new GI again. Should I insist on a scope, since it's been 6 years? Should he do other tests first (SBFT, endoscopy)? Should I get more bloodwork done? Any tips or advice would be appreciated.

I am pretty sure he will be scheduling you for another scope, and probably maybe a SBFT and definetly blood work. Since you haven't been scoped in awhile, I am sure he is gonna want to see what things look like in there for himself. I would also highly recommend you getting on some type of maintainence med and staying on them this time. The maintainence meds help to keep the microscopic inflammation down and stop it from doing its damage. Good luck and keep us posted.

If I go to the ER, will they do a scope then and there? I'm in a lot of pain and I don't know if I should wait to see the GI tomorrow, then have a scope weeks later (I cannot do anything the next two weeks). I have percocet from another thing but I'm wondering if it would be a good way to get a quick scope?

As for maintenance meds, no one ever even suggested staying on any medication last time. The new GI wasn't sure I had CD so he didn't want to prescribe anything.

No if you go to the ER they are more than likely going to do a catscan and xrays to check for a blockage. If things were bad and the put you in the hospital then maybe they would do a scope then, but when I have to go to the ER its generally a catscan thats done.

H i would ask for a scope and blood work to be done to see whats happening wishing you well hope you get on ok Bev x

Diagnosed with crohn's at 13 now 42
resection, right hemi 1993 In remmision from 93 till last sept with no meds or docs
2 resections small bowel  and fistula jan 09 and given temp stoma.  reversal sheduled for Nov09 Meds:Pentasa 500mg 3x day,  B12 shots, every 3 months, iron syrup,loperimide,folic acid, Back on citalopram 40mg (for Anxiety.)
