Inflammatory Bowel Disease Questionnaire: anybody have a copy?

I've been doing some reading and this has been mentioned as a measure that is fairly well correlated with disease activity, and I was thinking it would be useful (fun, to a nerdy person like me) to track my scores on something like this so I can see more gradual changes I might otherwise miss.

However, I can't seem to find a copy of the actual questionnaire and scoring ranges. I've got some articles on its development and questions but not how to score it, or an alternative form that has how to score it but not what ranges are considered "remission" or "active flare" etc.

Does anybody know of a measure similar to this, or a way I can get a copy of the measure without paying 150 dollars for a pack of 75? I'd like to use it to make a spreadsheet I can track my symptoms on. I can make my own but I figured one with norms and research behind it would be better than anything I can make up.

There is no such thing as Irritable Bowel Disease. There is Irritable Bowel SYNDROME and there is INFLAMMATORY Bowel Disease.

There is the Crohn's Disease Activity Index - which is a useless measure used in research. CDAI.
My computer says I need to upgrade my brain to be compatible with its new software.

Short Quality of Life in Inflammatory Bowel Disease Questionnaire (SIBDQ)

This questionnaire is designed to find out how you have been feeling during the last 2 weeks. You will be asked about symptoms you have been having as a result of your inflammatory bowel disease, the way you have been feeling in general, and how your mood has been.

I have to fill this out every time I go to the GI's office. Frankly, it doesn't ususally apply to my symptoms seeing as it only covers 2 weeks and it's months in between office visits.

It's interesting how patients are managed so very differently by physicians.  Hmmm, my GI is hard to get a hold of and I've never seen that questionaire before.  Wish there was a better gig in town! 



Symptoms since Jan 2009, Dx UC vs crohns May 2010 but probably small bowel Crohn's dz, biopsies all "normal"

Meds: Celebrex, Omeprazole, Liadla, Entocort EC 9mg/day, Vit D, Mulitvit, Omega 3's, Ambian, Neurontin
