IMHO you are showing sx of depression but I do feel a visit to your PCP for a good check up is called for too as you want to make sure the sx are not medical related.
In all situations that fuel your depression/anxiety, whether they be job related, family related, school related or otherwise, try and think about all the things that you have done right. While its true that it is a good thing to acknowledge things that you can improve on, beating yourself up over mistakes you've made will only increase your anxiety/depression.
It's really important for myself to be able to stay in the moment. I have found that by avoiding things like dwelling on past mistakes or worrying about the future I am usually able to get done what needs to get done when it needs to get done.
