Filling out Insurance questionnaire

My disability Insurance has cancelled my claim again, for the 2nd time now, and demand that I fill out a questionnaire, submit current SSDI status, go to an Independent medical exam and submit another continuing disability form filled out by my Rheumatologist.
On Dec 5, 2005, my Rheumatologist took me off work & hasn't released me to go back since, in fact he has deemed me permanently disabled. After 6 months my employer fired me. Disability insurance through my employer kicked in, and has been paying half of my previous salary.
They have been harrassing me constantly with requests for updates of my disability from my doctor. He has finally written on the form that I am disabled until 2057, because they will not accept the permanent disability status. They still keep asking for more updates.
My first attepmt at SSDI was denied, I filed an appeal, that was denied, then I filed for a hearing & since I didn't do it timely, it was also denied. My family suggested I get an attorney, so I did. I refiled for SSDI and promptly my insurance stopped paying me.
Now they say they will review my case once they receive all of the above. The IME is on March 23, 2009 & My bills are due.
Does anyone have any suggestions on how to fill out the questionnaire, which is similar to that of the social security form?
