End stage liver Symptoms & Developments

These are just some of the things that ESLD patient can experience or develop. I know that I am missing some others but can't think of them right now.

Some of the symptoms are:
Extreme fatigue
Itching
Edema
Confusion (encephalopathy)
Anxiety
Anger
Easily bruised
Bleeds a lot (hard to stop)
always cold
Jaundice

Things that can develop:
Infections
Ascites
Gallstones
Bleeding varices
Seizures
Strokes
Liver Cancer
Kidney failure
Death

I'm a new member and wondering if you can help me with what I suspect as end stage liver symptoms. My significant other of 23 years has NOT been diagnosed with cirrhosis but only because he refuses to go to the dr and to stop drinking. In Oct. 20080 he did agree to go for lab tests. The results:
ALP - 257 IU/L
ALT - 41 IU/L
AST - 97 IU/L
GGT - 884 IU/L
total bilirubin 2.2 mg/dL
direct bilirubin - 0.9 mg/dL

They didnt test for ammonia levels but I wish they had. He was not jaundiced in October, but he is now. His fingernails look yellow, the whites of his eyes and skin. He is always tired, doesnt sleep well, continues to drink wine although he has tried to cut back. His liver is visibly enlarged and confirmed by the dr. he appears to have ascites, he cant eat much and as soon as he does he must lie down and take a nap. He says "he's fine". My question, does he appear to be in "end stage" liver disease based on his lab tests and physical symptoms? He has also had a chronic sore throat for the past 4 wks. He goes through bags of cough drops, even getting up in the middle of the night for one. He does NOT have any health insurance and when I mention applying for Medicaid, he refuses to listen. He is well educated, intelligent man. He mistrusts dr's and the medical community in general. I am preparing myself for the worst. Is there anything I can do? How long can someone survive in this state without getting help? Is there a point when he will need to be taken to the hospital anyway? Thanks for your feedback. Any help, suggestions, comments would be appreciated.

Hi, Damselfly, and welcome to the forum.  I am sorry to hear about your fiance's problems.  He probably doesn't want to go to a doctor because he knows they wll tell him to stop drinking.  If he's not willing to stop entirely, he will only continue to get worse and nothing that is done will help.

It appears that he does have liver disease, but it is difficult to say whether it is end-stage or not, as we are not doctors and each case is individual.  Some patients will continue on as he is for years and others will get progressively worse more quickly--particularly if they continue to drink and do not eat a diet rich in fresh fruits and veggies.  Yes, it is possible that you will one day have to call a rescue ambulance to take him to the ER for a bleed or coma or encephalopathy.  I suggest you read as many of the educational threads on this forum that you can, to familiarize yourself with liver disease and all that it entails.
