Hello again, I got another question for you all! I am seeing a new rheumy and she has ordered some blood work....
 
 -CBC diff platelets
 -Comprehensive Metaboloic panel
 -ESR
 -ANA ds DNA, SS-A, SS-B
 -C3 C4, ENA
 -CPK
 
Is there more lupus specific tests other than these? I thought anti-sm was very specific to Lupus, which was not listed.
 
What tests do your doctor's run? Just curious.

Ya, I think you're right KaAl.  Anti-sm, Anti-ro.  Looks like she has everything else.  Except the APS test! I mentioned that in another post to you....
 
Another one to get tested, and they might do it automatically with the APS test, is the anticardiolipin antibody. 
 
I"m glad your new rheumy is getting going on all this for you.  It's about time, right!!!!

The ENA panel usually comprises anti Sm,anti RNP, maybe Jo1, Scle70. Anti Ro is SS-A and anti-La is SS-B.
The so called Lupus Anticoagulant clotting tests would normally be done along with anticardiolipins and sometimes beta2 glycoprotein to indicate autoimmune disease activity, rather than infection. RF is often done too.

O.k, so the ENA is actually te anti-sm and anti-ro test? Sorry I have no clue.

Hello

It looks as if she has specified anti- Ro(SS-A) and anti -La(SS-B) separately, I wouldn't know why. The ENA panel presumably will be anti RNP and anti Sm and maybe the other two mentioned in previous post. It rather depends from lab to lab. Maybe two different test methods for some reason, so anti Ro and anti La would be done twice. Maybe she thought first of them and added ENA as an afterthought.
The main thing is that it seems that all the usual is being tested except anticardiolipins and the clotting tests for 'lupus' anticoagulant.

By the way anti Sm is very specific but not very common, only about one third or less and unusual in Caucasians

Most labs won't expand to the ENA unless the ANA is positive.  My Rheumy orders some of the antibody tests seperately because my ANA is not positive all the time.  I have the Scl-70 anitbody that stays positive.  He is watching the levels of it.  If he didn't order it specifically, they wouldn't test the Scl-70 if my ANA was negative.  Make sense?
