Hello all - hope you can help me figure out whether what I experienced this week was a lupus flare or something totally not autoimmune in nature.  I've been seeing a rheumie for 1 1/2 yrs.  She keeps testing me for everything autoimmune every 6 mos.  The only consistency is a positive ANA - which keeps going up.  Last one in May was 1:1280.  After trying to ignore some impending flare signs, I awoke the next morning with fever, wicked h/a and pretty severe joint/muscle pain.  I hurt in places that hadn't bothered me for awhile.  Took Advil for fever and pain.  It barely took the edge off.  Lots of fatigue and spaciness, but I also hadn't been sleeping well.  I have fibro and CFS as well, so pain, fatigue and spaciness are not strangers - but the fever (102.6 at the highest) and weird headache (around the eyes & eye muscles- with little head rushes if I moved too fast) were specifically not the norm. 
I came down w/ a big nasty virus in 1997 - never really dx'd beyond EBV and CMV.  Fever for 3 weeks - rash, fatigue, jaundice/hepatitis, muscle weakness, joint pain, weight loss, etc.  It started with a high fever (104) and the worst headache of my life - also w/ specifically painful eye muscles.  I had to quit my job and was never quite the same - the convalescence leading into the fibro and then CFS.  Had a recurrence in 2005 of a similar but milder version - only lasted a week - but same hallmarks of fever, headache and joint pain.   It seems to me this is what happened again last week.  No other symptoms usually assoc. w/ a viral infection....no sore throat, congestion, cough, diarrhea, etc.  The fever was gone after 4 days, with all symptoms reducing in intensity after 2-3 days. 
GP ran West Nile, and metabolic panels.  So far, liver enzymes were pretty high, CRP through the roof, incr. white cell count and positive ANA, but lower than in May.  Still waiting on hepatitis and West nile results.  Does this sound like a typical lupus flare?  The rheumie suspects lupus, but blood doesn't confirm it.  I don't have malar rash.  Thanks for listening.....can't seem to find info on flare symptoms....maybe they are all different?

Lucy, you are right that a flare can be a bit different for everyone, but what most people say is that their symptoms get a lot worse and/or they get symptoms back that maybe they hadn't had for a while. Something is going on with you and I'm glad your GP was very thorough. I've had high liver enzymes at times too and was checked for hep - there is also and autoimmune hep that is related to lupus. Sometimes for me, it can be hard to tell if I have the flu or if my lupus is flaring. I hope your doc can give you some answers. You've been through a lot these past few years and being in diagnosis limbo can be really hard. Let us know what you find out about the rest of your labs. ((((hugs))))

Meds:  Plaquenil 400mg; Prednisone 5-10mg; Tramadol 100mg 3-4x daily; Doxepin 25-50mg; Prevacid; Evoxac; Steriod Cream and Mouth Rinse for tongue and mouth ulcers; Hydrocodone 5/500 prn for severe pain; Artificial tears

Lucy, Hippi gave you some great info and advise. I have nothing to add just wanted you to know you are in my thoughts and prayers. I pray your doctors figure this out soon, I remember how frustrating the dx process can be. Take care and keep us updated on how you are doing

RX: Imuran, Plaquenil, Methotrexate, Flexeril, Piroxicam, Levoxyl, Folic Acid, Lortab (as needed) and magic mouthwash (for mouth sores)

Thanks, Ladies - it is hard to put all the puzzle pieces together.  I even feel for the docs! I appreciate the advice and info. (and prayer!)  Maybe it was just a bug....I've been feeling much better.  And my experience w/ fibro/cfs flares is that they don't just turn on and off.  Also - had my first acupuncture treatment 2 weeks prior to whatever this was, and had been drinking the herbal concoction the TCM/acupuncturist sent me home with, and had radically changed my diet.   Wondered about a reaction to that - though he didn't believe it was a Herxheimer's response.   Will keep you posted - thanks again!
