mtx/methotrexate question

Tonight my husband starts mtx at 10mg could anyone tell me what to expect? He also begins folic acid because he already has mouth ulcer issues. Actually he has ulcers right now. I just want to know what I can expect as far as a reaction to the meds goes and also if there is anything I can do to make this easier for him. THANKS!

I have been taking MTX for 8 months.  You can divide the doses in two doses 8-12 hours apart.  I don't have any problems with it.  Maybe a lil bit of nausea but nothing bad.  Good Luck

Hi redrose,

I take 10mg and its not bad at all (for me). The first week I had a bit of nausea the day after I took it and I still get a weak feeling from time to time but I try to take it easy anyway. I take it during the weekend so I can just kind of relax if I need to. I still am able to do errands if I need to and I tidy around the house a bit, even go out to lunch or shopping with a friend (that all depends on the lupus, not the med). It doesn't keep me bed bound if that is the concern. I have been taking it a few months though and my body has learned to tolerate it. My doc says not to take the folic acid the day I take the MTX but I take it the other 6 days and it helps with the side effects. I may get a mouth sore or two but by Monday its cleared up (I take the MTX on Friday evenings).

thanks everyone. I will let you know how it goes. We are actually hoping this doesn't go well so he can just do enbrel our insurance requires a trial of mtx first. Thing is eventually we want to try 1 last time for a girl and mtx is something he has to wait 90 days after stopping before I can get pregnant. Given how remission can be unpredictable we really need to be able to try when my next remission comes. I know everyone is wondering if I am insane but I want my son to have someone close to his own age to grow up with. Being an only child is lonely as both my husband and I are aware- our siblings have such an age difference we both were an only child for the most part.

I am doing very well on mtx. The first two weeks were difficult then I went to 3mgs of folic acid per day and never had another side effect. Plan on him being a bit more tired on the day after the mtx. This may be even more true if he did not start the folic acid several days prior to starting the mtx. The mouth ulcers may go away completely once he has been on mtx for a while.

I have been on mtx for about two years now, maybe longer.  I'm currently taking 15mg/week, which is probably my highest dose--I won't go higher.  I started off on 7.5mg/week.  At first, I had some issues with fatigue and nausea for a day or two after I took it, but now I don't really experience many side effects at all.  And it works wonders for me.  I was scared to take it for the longest time, but it appears to be the best drug for me.  I also take enbrel, but could ditch the enbrel, stay with the mtx, and be fine.  If your husband has some issues with it, perhaps he can try taking it at bedtime for the first few months.  That's what I did, and if I experience any side effects, I end up sleeping them off.

Thanks. His first dose appears to have gone well. I wish we would see results sooner than 2-3 months but he will try and deal with the nausea until it goes away. He takes his meds at bedtime but still didn't sleep through all the nausea from the first dose. I wish this was easier on him.

Good Morning Redrose77,
I was just recently diagnosed with Lupus and was put on the methotrexate about a month ago. My rheumy took me off it last week because I got a severe upper respiratory infection and had to put me on a ZPAK. I'm not sure if it was really helping or not, all I know is that my immune system was so shot I couldn't fight anything. Got pinkeye as well while I was on it. I'm still doing alot better than I was 2 months ago, so I guess that may be from the prednisone, plaquenil and short term use of the methotrexate. All I know is that they say you are prone to getting more infections while on these disease modifying drugs, which of course stinks because you already have issues with the autoimmune symptoms. Last thing we need is to deal with other infections. Tell him to try and stay out of contact with anybody who is ill if he can, especially now with all this Swine Flu stuff that's out there. Hope he sees results real soon.

Hi Redrose,

I'm sorry hubby is feeling nauseous from the mtx. I was taking 20mg (started at 15 mg) and never felt sick. I was fully expecting to be ill. I had cleaned and disinfected the bathroom, knowing I'd be sick . . . but it never happened. I did feel sleepy for a day or two after taking it.

I also never got sick (like colds and flu and such). I think when we need these types of meds, our immune system is too active to begin with . . . so it gets leveled off. I was careful about being around sick folks . . . except my grands . . . I take them any way I can get them . . . LOL.

I hope he does better with the next dose.

Thanks. I really hope the next dose is better too. At the same time we really would prefer he be on something else. It sounds weird but this is a drug that scares the heck out of me. I know a lot of people take it safely but truthfully I hate taking risks with his health. Any risk.

You know if the nausea continues to be a problem for your husband, mtx is also available as an injection.  Many people with IBD, and hence lots of stomach issues, take the injectable mtx.
