Watch your chart notes, heres a laugh
 
Well I had to share this.... We all need a laugh!
 
 
Doctor's charting...
 
Collection of documentation statements actually found on patient's chars during a recent review of medical records. These statements were written by various health care proffessionals including some doctors at major hospitals.
 
 
The lab test indicated abnormal lover function.
 
The baby was delivered, the cord clamped and cut, and handed to the pediatrician, who breathed and cried immediately.
 
She stated that she had been constipated for most of her life until 1989 when she got a divorce.
 
I saw your patient today, who is still under our car for physical therapy.
 
The patient lives at home with his mother, father, and a pet turtle who presently enrolled in a day care three times a week.
 
Bleeding started in the area and continued all the way to Los Angeles.
 
She is numb from her toes down.
 
While in the emergency room she was examined, x rated and sent home.
 
The patient suffers from occasional, constant, infrequent headaches.
 
Coming from Detroit, this man has no children.
 
Patient was alert and unresponsive
 
When she fainted, her eyes rolled around the room.

Dx:fibromyalgia 2002, systematic lupus 2005- definate CNS involvement dxed late 2005, psoriasis 2006, rheumatoid arthritis 2006, PTSD 2007, multiple allergies 2005, migraine, compression fractures T11 & T12, Sjögren's, damaged periphrial nerves 2007, exema
Tx: plaquenil, Enbrel, Tramadol, Singulair, Skelaxin, Baby Asprin, Imuran, Prilosec, lasix, Evoxac, Celebrex, Darvocet when things get too bad, prednisone again, various vitamin/mineral supplements, cozar

Thank you so much for that.. I really needed it. Especially now. Our dearest and oldest friend whom i called Uncle John passed away from a bleed out. He had an aneurysm of the bowel had surgery, was put back on coumiden, We think he over did the walking which possibly pulled a stitch i don't know. He's gone and my dad has lost his best friend in the world. cry We will all miss him terribly.
