lupus information check list

I am looking for that check list to take to the doctors. I saw it on here when I first started and now I can't find it. It was a detailed list of everything you need to take to the doctor. I am being referred to another specialist on the 28th and I want to have all this information ready.

Does anyone know where I can find it.?
DX: Lupus(UCTD), Asthma, Mitral Valve Prolapse, Heart Murmur, Anemia, Vitamin D def., B12 def, Hypertension r/t lupus, TIA, muscle weakness, numbness, tingling, headaches, night and day sweats and IBS.

Meds: Imuran 100mg, Prednisone 60mg, drisdol 50,000 units weekly, b12 injections monthly, Protonix, Zantac, Tramadol 4 times daily prn, Align, Kapidex, Amitza, Miralax, Vaseretic/HCTZ Aspirin.

Allergies: Cipro and Bactrim
I can do all things through Christ that strengthens me.

My eye site must be failing me because I can't find it. It was a printable list to take with you at each doctors visit.

Symptom List

This sheet should be typed carefully on your computer and then updated for each appointment. It takes a fair bit of time to make the first one, but then you can just update it for each following Doctor appointment.

You'll always take two copies with you, so you can hand one to your Doctor. Most doctors love it because they can get caught up very quickly with this list and they will usually keep it with your file so they don't have to re-write every item. You can keep your extra copy in your wallet in case of an emergency, you'll have all your medical info handy.

I think so, but I thought it was a little bit more detailed but this will work. Thanks!
