ANA/Smith antibody???

Hi everybody. I need some help. My PCP ran routine blood tests that came back with a positive ANA result. I'm negative for each anti-body tested, except the anti-Smith (sm) one. Negative is 0-0.9 and mine was 2.1. There wasn't any issues with my urine, but he said my white blood count was slightly high or low (don't remember). Nothing on that was flagged on the report. I'm 30 and don't think I have any symptoms except being tired and maybe a little joint pain. I assumed this was due to horrible sleeping habits and the lack of exercise. I've read that one needs to have 4 out of 11 characteristics to be officially diagnosed with lupus. However, I've also read that a positive anti-Smith result is pretty conclusive even without symptoms. I can't get in to see a specialist for a month and just want to know if anyone has any info. about this anti-Sm result. Does this mean I will probably show signs later in life and be diagnosed then? Please, help.

Unfortunately, there isn't anything that ties directly to lupus -- as you said, it takes 4 of 11 for diagnosis and there is no data that determines any relationship to this now and what symptoms of diagnosis may come up in the future. It's a wait-and-see thing, which I know is not the most reassuring of answers.

I hope you are going to be seeing a rheumotologist - those are the folks who know most about anti-immune diseases.

Hang in there, and don't worry - use your energy for something fun instead!

In response to new member. I too am a new member. You have a positive ana result, joint pain, fatigue and a positive anti-sm result (which is highly specific for lupus), that means you have 3 out of 11 critria. The rest may certaintly come in time. They will also take into consideration your medica history. Make a list of everything you can think of. A good rheumie will listen and make the proper diagnosis. Good luck.

Thanks Teresa. This is scary and I am very sad and anxious to find out. It seems that, as long as the tests weren't mixed up somehow, I will one day show stronger symptoms and then be diagnosed. Is a positive anti-sm pretty much a forecast of a future of SLE?

HEY i JUST JOINED this forum. I have a high anti smith as well ans meet with a Rheumatologist tomorrow. I was wondering what your DR said. I see you went about a month ago you stated Any news??? I know its frustrating my only symptoms are chest pain , knee pain and 2 episodes of blurred vision.. and the positive ana with high smith antibodies

the appt went well. he didnt seem concerned about the chest pain . he ordered more bloodwork. a smith antibody again and a ana with titer and a urinalysis. i guess we will go from there. i get results tomorrow thanks for the concern!!!
