I'm assuming you're referring to a defogram? It isn't painful at all. It's a little humiliating but the radiologist and techs that did mine were quite professional so my embarrasment was minimal.
They will have you lie on your side and put some tubefulls of barium paste into your rectum, you may need to drink some barium first and wait 30 minutes (I did). Then they have you sit on a make-shift toilet with your side against the xray table and the radiologist tells you to hold the paste, let it out, etc...It only took about 15 minutes and my radiologist let me watch the whole thing on the screen and told me about my rectocele. Then they let you go "expell" the rest and clean up.
