Are you a relative of someone with Alzheimer's Disease? If so please go to http://www.alzheimerssurvey.vze.com. The survey is quick and anonymous. I'm glad that someone is trying to collect info and turn into something of good use to us. The quetions were simple and the form was easy to use. It just took a minute or so.
