CBT instruction?

Several posts mention CBT instruction is somewhere on the forum, but I can't quite find it.  I've found descriptions of what CBT is, but not worksheets or how tos or anything.  Is there such info within the forum somewhere, or are they saying to get a book or something?  Thank you.  I've had major depression for 30 years and bad anxiety/panic for over 10 years.  I have seen therapists, but they have never been much help.

Been living with Crohn's Disease for 33 years. Currently on Asacol, Prilosec, Estrace, Prinivil, Diltiazem, Percoset prn for pain, Zofran, Phenergan, Probiotics, Calcium  w/Vit D, and Xanax as needed for my anxiety.  Resections in 2002 and 2005. Also diagnosed with Fibromyalgia, Osteoarthritis, and Anxiety. Currently my Crohns is in remission, but my joints are going crazy!

Nanners posted the link for CBT for you.  It is a free online program and you do your work right online.  It may start out a bit slow as you go through a lot of questions but then you start working on your own issues.  Do give it a try.
 
There is another self help workbook you may be interested in.  The Dialectical Behavior Therapy Skills Worrkbook by Matthew McKay, PH.D.  You do have to order this one and I know Amazon.com has it.
