The first one approves the work of the stimulator representative's work. The second one approves the surgery to implant the wires.

The stimulator rep will be there in the surgery room during the trial implant to test ("analyze") the unit to make sure everything works okay and is safe. They will also set up your initial programs for you so you can begin using your trial unit that same day.

Typically insurers won't approve the permanent implant until they know you had a successful trial.

Good luck!

Hey Stumped, that is great news! I wish you all the best and hope you have success. I don't know anything about the stim except for what I have read, but it does sound like it can be a god send for some. I sure hope you are one of the lucky!

Take care.

Congratulations Stumped.......
Someone said that the paperwork is for the approval for the trial. They normally send the approval for the stimulator trial first, and depending on the type of leads being used during the trial, they may send the approval for the implanting of those as well. If you are having paddle type leads used, then the implant for those is done differently than the round multicontact leads used during most trials.
That may be why they are listed on the approval form.
Now that you have that, you can set your date with your doctors office.
Good luck with the trial, I hope that it brings you the pain relief that you seek.
