We are not doctors here so no one should be recommending medications or dosages. Your best option is to discuss your concerns with your prescribing doctor and let him/her offer you whatever options he feels are best for your conditions.
Most pain management doctors use a combination of medications and other treatments modalities to help treat the pain.
Very true! But we can suggest medications and dosages so that someone can discuss that with his doctor. Doctors are so busy that oftentimes they don't think of all the options available when it comes to medications. I have the same issues with my lower back that this man does.
One other thing I do that I didn't mention is I get off my back regularly. I try to remember to set a kitchen timer for an hour, and that's all the time I give myself to stand up and/or sit down since both aggravate my pain. Then I lie down on my couch or whatever for at least ten minutes if I can. That helps considerably because the back gets to rest.
