I'm sorry you're going through this. I had a bigger review once a few years in. I was reviewed every year after but it was just a form saying they had done it and my benefits would continue. This was several years ago so some things may have changed.
I received a letter last year saying they would no longer be doing reviews.I would get a copy of your health records, read them and send them in with the paperwork. Go into great detail on any questions. Don't just say I can't work now. Explain why you can't whether its pain or fatigue or whatever.
Tell them what happens when you do do something you shouldn't. Add letters from all Dr.s especially the one who DX'd you or who is taking care of those issues. I had one from my Neurologist and my GP. If your health problems have progressed make sure you explain that and what the progression is. And, I would add in depression as long as you have a someone who can verify it. If they don't want it it's no big deal. I even typed a letter basically telling them that I can no longer or do office work because I had become dyslexic. I told them that I didn't correct any of the letter so they could see how bad my problem had became.
