How to switch PM doctor?

My PM doctor is moving to another location . Plus I don't care to much for him anyway and figure this would be a good time to switch. What do I need to do to make sure this goes smoothly? Im going to need my meds filled towards the end of January. Im worried about that.
Any ideals?

Ask your current doctor's office for a copy of your records, you will have to sign a release form and you may need to pay a small fee for them, but they are yours for the asking. Take said records to the new doctor you choose.

Plus you may need a referral to somebody else . ask your reg Doc to do that . Your reg dr. should prescribe your meds till you find a Pain guy
