phenergan should only be administered IM or as an IV piggyback. Meaning the medication is put into an IV bag of fluid. I'm an RN and our hospital standard is the phenergan must be mixed with at least 10ml of saline for IV push. I never do IV push as I'm aware of the risk involved. I always mix phenergan in a 50ml bag of saline for slow infusion. I myself had phenergan infiltrate a vein in my arm. Luckily my tissues did not die but my arm was very sore for a month.
