Here are some things that have really helped me:
1. studying by correspondence. Find an institution that offers self-paced study, as you don't want to be tied down to deadlines. It's also important to ensure that the course won't require you to get out of the house and interract with people, as this can be tricky when your Crohn's is uncontrolled.
2. Pen Friends! For years, I've been a member of this pen friend club
and it has really helped to reduce the feelings of isolation. A lot of people who apply to become pen friends are isolated or unhappy themselves, so you're likely to find some kindred spirits that way. You can also find pen friends on the net, if that helps.
3. Find a safe place that's near your home, that has a toilet and understanding people, that is fairly easy for you to get to. For me, it's the library: three blocks away, with a toilet, with lovely staff. I can't get there when I'm incredibly unwell, but it's the first place I go to when I can leave the house and am desperate for company. You might be able to find a cafe or a nice shop or something like that near you. Just getting out of the house helps reduce feelings of isolation.
