HELP! Crohn's and Colitis Survey!!

Hello All! 
I really need your help!  I am a graduate student with Crohn's Disease (27 years) and am doing my Master's Thesis on Self-Efficacy(confidence in completing a task) and those afflicted with Inflammatory Bowel Disease. 
I need 50 people to take a few minutes to fill out my survey online at the link below. 
IMPORTANT!!!  You must be 18 years of age or older and have Crohn's/Ulcerative Colitis/IBS.
The survey is completely ANONYMOUS. 
It is my hope that the results of this will help shed light on this very important factor in coping with these diseases and our ability to manage it.

In so far as your survey is concerned, I think you need to approach through the forum administrators. Thats what I feel. Please read the forum rules.

The design of the last question could skew your results. Someone with a new diagnosis may not have had a treatment or remission, none of the available choices cover this.
Diagnosed July 2009.

Currently taking 2g Pentasa twice daily.

Waiting for first doctors appointment to get some information and treatment, been threatened with surgery already, but hoping to avoid this through some sweet talking with my Doc.. when I meet him.
