Oral antibiotics have never helped my RV fistulae in the past. I'm glad it's eased a bit, but I am pretty concerned. I had one that burst on the inside and I went septic; it was pretty awful. If it still stays painful, etc. then I would go to the ER.
opening it surgically just depends. My first one they just did a lidocaine shot and cut it in the ER, but then the ER doctor freaked out because it was a lot deeper than he thought (he thought it was just from an ingrown hair or something stupid like that). They packed it but didn't send home any packing with me, and it only healed on the outside and re-formed and burst through into the vagina and that's how I got my recto-vaginal fistula that hasn't healed, and that was over 4 years ago and I'm on Remicade.
The second time, my GI was gone and I didn't have a surgeon yet (I'd only lived in that area for 6 months). The PCP told me it was just a swollen gland because I didn't have a fever (I never get them) and sent me home. It got too painful that night so the next morning, Saturday, I went to the ER. The ER dr called a surgeon and they did it under anesthesia. I was admitted for observation that night, which was good because I got a sudden fever of 104 and it turned out I'd gone septic. It took 5 different IV antibiotics all at the same time for days to get things under control. But they showed me how to pack it and I had my husband pack it every night for about two weeks so that it healed from the inside out. I've never had any trouble with it.
Sepsis is rare, so please don't let me freak you out, but if you get a fever, go to the ER immediately. And make sure that with Flagyl you take a ton of probiotics so you don't get c. diff. I personally like Florastor because you don't have to space it apart from the antibiotics and it's more effective than regular probiotics because it's yeast-based. I take 4 per day while I'm on antibiotics.
