I had one.
It didn't hurt at all.
You go in, put on a robe, and then someone comes in and tells you what's about to happen, i.e. they are going to inject a dye into the fistula to see where the fistula track goes.
I was told to say something if I started to feel any discomfort because he didn't want to aggravate the fistula.
This procedure was the very first thing my GI ordered after my first visit with her. My fistula was only 3 months old at the time. It showed her the extent of the tract, information which was sent to my colorectal surgeon, who then determined that an invasive fistulotomy surgery was not an option for me.
