Hello All,

Over the last 2 days I've had almost constant rectal pain. It's worse after a bm. Hard to pinpoint exactly where it originates from. At times I think it's near my tailbone. Other times it's the lowest point of rectum. I started amox-clav last night (with florstor) thinking and fearing its an abscess. The skin portion of the fistula has closed but it's tender to the touch.

I had no pain this am until after my only bm and it hasn't relented.

Questions:

1) Is proctitis a constant pain or does it disappear within a period after a bm?

2) Can I use proctofoam enema if I have an abcess? I know it can numb and help inflammation but I'm so afraid to irritate whatever is going on. It feels different from any previous abscess but it's already annoyingly painful and obviously I don't want that to worsen.


I'm having difficulty sleeping and obviously sitting. Still getting on with life though but am very fearful of another torturous abscess. I'm using 1/2 percocet a day to take the edge away but I should probably be using more.

On another note; this couldn't be worse timing wise. I have a colonoscopy in 2 weeks. It's only if I'm in clinical remission that I can enter a stem cell trial or have a fistula surgery. I'm tired of these _______ fistulas! After 2 years I need them fixed as they still cause me issues. Considerably less so since Stelara but any abscess is traumatizing.

Bowel is good but softer stools and minor urgency (at times). Tummy and gas due to abx.

Oh my goodness. You've got enough fistulas already on your plate U B Tough. Can you see a colorectal surgeon? Proctitis pain is shooting pain up the rectum and I can have severe spasm. With proctitis, you have tenesmus where ypu keep rushing to the toilet w/o being able to pass anything out. For me it's not related to bms. I can have it any time esp. when I'm sleeping. It wakes me up. I don't remember that either my GI nor the CR surgeon allowed me to take any enemas when I got an abscess. With my perianal fistula, I can only take colifoam as any liquid enema like pentasa for example would come immediately out of the fistula.
Can you take the antibiotics in a suppository form? This is actually what I'm doing now. My proctitis is flaring up and I asked my GI to allow me to take metronidazole suppositories /12 hrs for 5 days. Out of experience, this eases rectal spasm and inflammation.

Thank you Minnie. I can't be bothered seeing the CRS. Kinda burnt out and I don't want an exam as it's too painful. It kind of eased but today I woke with burning in my lower rectum. It feels like something is right at the exit. Wondering if it's a hemorrhoid? Haven't had one before. I've just inserted some ointment to see if that would help. So far no difference.

I didn't know you could get abx in a suppository form. I see my GI for colonscopy on the 30th and hope I can hold out until then. Still using 1/2 a percocet at night and just trying to distract myself as much as possible.

I don't have tenesmus and no b, m. Only going once a day, thankfully.

I have Stelara tomorrow so hopefully that will east things.

Do you think you'll try Stelara? It's been amazing for me. I just don't understand why over this last year especially since my bowel is good surgeons don't do fistula repairs here. I'd have to go to the states. I researched a place in California. The cost of the surgery is reasonable, it's the 2 week hotel stay, food, airfare etc that's too difficult. Not to mention I'd have no help.

I was also wondering about tacrilimus suppositories.

I think I will try a foam enema tonight. I'm so hoping it's not an abscess....
