Hi, I supposedly have Ulcerative proctitis, but my rectal ulcer was fistulating. I know what you mean about that spasm feeling, I get it often, I hate it! My doctor told me it's called Tenesmus. I have had this same rectal ulcer for at least four years. My G.I. thought it would be easy to "fix" with rectal meds at first. The only thing that has worked is Remicade, which closed the fistula but the ulcer is still there. The only thing I have found to help the outer rectal pain in Lidocaine Viscous 2%, it is weird to use cause it's a syrup, but it is the only thing that doesn't burn like mad and helps! I would love to talk to all of you whom are going through the same things and share ideas/experiences/advice. Sorry you all are going through it, I know how painful and hard it is!

Tenesmus is the constant feeling of the need to empty the bowel, accompanied by pain, cramping, and involuntary straining efforts.
Tenesmus is generally associated with inflammatory diseases of the bowel, which may be caused by an infection or by other conditions. Tenesmus is characterized by a sensation of needing to pass stool, accompanied by pain, cramping, and straining. Despite straining, little stool is passed.
