Going back to work after depression: insurance company is making sick

Going back to work after depression : the insurance company approves 2 days weekly for 3 weeks.
My doc says 2 days weekly for 3 weeks followed by 3 days weekly for 4 weeks.
My boss is putting pressure on me : all of a sudden, everything needs to be done, she's adding tasks that had been assummed by other people and yesterday, when informed of the doc's return schedule, could only say that she would have to verify if the insurance company will accept this progressive return, not saying what action she would take if they don't.
The insurance company has a policy that once you are considered able to work, you return to work, full time, right away. Allowing 2 days weekly for 3 weeks was very accommodating on their part, so they say.
I already suffer from anxiety, social phobia and all these people are even scarier.

- I'm scared the insurance will refuse my doc's return schedule. If so, they will withhold my salary.
- If they refused, my boss can not accept to accommodate a part time job, even for a few weeks.
- I'm scared I will loose my job over my physician's prescribed return schedule. My job is not covered by a union.
- I'm afraid my boss will go on being adamant about my taking all my previous responsibilities and taking on new ones right away. She has never recognized my job is too much for one person.

I'm so anxious I can only think about this. I honestly do not know what to do. Referring the insurance to my doc sounds impossible this time. I saw her yesterday but she was obviously sick and I'm scared she will take a leave of absence: she has done that 2 years ago, gone for a year. Psychiatrists are allowed to be sick but no one will be able to take on her patients list any time soon.

(Please don't be offended if I post the same in the anxiety forum: I'm feeling so helpless and I do not know where to turn)
