the patients getting the newly approved drug combo tx of sovaldi + riba. i am not surprised that the new double and triple combos are starting to get prescribed. competition from the other 5 or 6 pharmas are close. this year maybe. bc/bs and medicaid have added sovaldi to their formularies. medicare advantage also will follow suit. check with your insurance, and if there not ready, your dr can document your need for the tx asap.
So happy to hear a couple of our new menbers researching and advocating for these new txs. It is necessary to stay on top of what has been approved and for what genotype. Just to review what has been approved for prescription in the last 3 months;
geno 1a or 1b, and geno 4; sovaldi + interferon + ribavirin for
12 weeks. and must be tx naieve
geno 2; sovaldi + ribavirin for 12 weeks
geno 3; sovaldi + ribavirin for 12 weeks
geno 1 or 4; olysio +interferon + rib for 12 to 24 weeks
genotype 3 sovaldi + rib only has a 30% sucess rate, while adding 4 weeks brings it to a 62% sucess rate. interferon /rib for 24 weeks has a slightly better svr rate. 24 weeks with sovaldi is best.
for genotype 1 and 4; drs have been sucessful prescribing olysio + sovaldi for 12 weeks off label for tx experienced. for those who are interferon intolerant and very sick, this is being done at normal copays. it absolutely makes no sense for gilead, Abott, BMS to hold out for higher pay days. gilead has a head start and has to make up $11 billion dollars for the price of acquiring sovaldi from pharmasset. no one can afford sovaldi without ins coverage. they need sales NOW. Gilead is also starting trials again for sovaldi + daclatasvir, as they had the best preliminary clearance rate for geno 1 to date. Remember gilead broke off their collaboration with BMS 2 years ago, because they didnt want to split profits on sov + dacla.
when a dr says sovaldi doesnt need ribavirin, what are they going to replace it with. Pharmasett tested sovaldi as a monotherapy in 2009 and the best results were 60%. i dont remember for which genotype, but it is marginal on its own. good luck to everyone getting what you need. as Allen and Music said, research and be armed when talking to your drs and insurance. dont take no for an answer. it is available and it is a crime for any one to slip thru the cracks.
