Husband referred to nephrologist - new at this!

We are usually in the lyme forum as both my husband and I have lyme and he also has babesia, a parasitic coinfection.  My husband has been having frequent urination, groin pain, getting up in the middle of the night for few years now.  A recent abdomenal MRI found the following (prior to the above diagnosis):  bilateral lobulated kidneys, suggestive of scarring; multiple pararenal cysts of left kidney; parapelvic cysts on upper pole left kidney.  All of his pain has been on the left groin and side.  When hospitalized a few months ago, they indicated his kidney function blood work was abnormal and now with treatment for lyme, it has gotten even worse and is being referred to nephrologists. 
 
Any advice on what to expect or does any of this sound serious? I don't know if from the infections or if cysts or scarring are common or what.

Lyme, HME, HGE, bartonella. 4+ years undiag. despite 10 drs.
Current meds: roxythyromycin, bactrim, doryx, amanatadine, neurontin, cymbalta Prior meds: amox, minocycline, tetracy. IV refused by ins. Supplements: boluoke, alpha lipoic acid, AG immune, magnesium, milk thistle, resistant microbes
