This survey is to collect information to find the major causes of kidney stones. Kidney stones are usually made up of a compound called calcium oxalate which is a chemical compound that forms crystals in the body. There are no costs to participate in the survey. The survey is open for all people no matter what the age or health history is. By taking this survey, it is contributing to find the common cause of kidney stones. The survey will only take about ten minutes to complete.
This is a web-based survey so no IP addresses will be collected. Nobody will be able to identify you, your answers, or even if you participated in the survey. The survey is anonymous, however, absolute anonymity cannot be guaranteed over the internet.
It is your choice whether or not you want to take the survey. By completing the survey, you are agreeing to participate. You do not have to answer all of the questions if you don’t want to.

i have been diagnosed for kidney stone 8 months ago.. i can understand the pain and the problem one feels
