Hi. Very confused. I have a hx of neurogenic bladder, bladder augmentation surgery, and do self cathing. Have had occasional issues w/ kidneys in the past.

So, went to see the doc about a variety of symptoms. I'll spare you all the details, but basically they determined it was a kidney infection. Started me on Cipro, said if I don't start to feel better after 3 doses of antibiotics, would need to be admitted. Doc said based on symptoms, the infection had "spread into my blood". What? I never heard that before....what does that mean? I guess I had thought that since the kidneys filter the blood, any kidney infection had therefore gotten into the blood.

Please tell me, am I confused or is my doc?

One more question, I am pretty sure I should have gone to the ER last night, but I didn't. I was hot then cold then back to hot 100 times an hour. I was confused. Back pain, headache, nausea, even a little chest pain (like someone was grinding their fist into my sternum). Positive I wasn't drinking what I should've been. Ok, I guess I probably should've gone, but how do you know what's safe as far as when to go to the hospital and when to "tough it out"?

Thanks for any info you have.

General rule of thumb: if your temperature is above 101 oral (a degree lower for axillary or forehead), if you have new signs of blood in the urine, if you have nausea or vomiting or aren't producing urine, go to the ER.
Kidney infections don't normally include septicemia, and that is a serious, potentially fatal, complication. You are also putting your kidneys at risk with inadequate treatment. As I'm sure you know, antibiotics are becoming less and less effective as common pathogenic bacteria become resistant to them, so the Cipro may not be working for you.

Please do call your urologist today. It could be a matter of life and death.

Thanks for the info.

The doctor I had seen earlier in the week was an internist. I was confused about my symptoms and the internist I saw apparently also works 1 day a week seeing patients in the incontinence and sexual health program, so I figured it was something she could handle. Guess not. The internal medicine clinic was totally disorganized. I have the contact info for the manager of the clinic who I will be calling next week.

I will definitely talk to my urologist today. I'm still feeling pretty bad, but quite a bit better than the past few days. My urologist is gonna be mad! Mostly at the internal med clinic for the way they handled things, but also at me for not getting him involved several days ago.

Currently waiting for the clinic to call me back about making sure i'm on the right med. I'm guessing I am because I am feeling better, but gotta be sure. I'm giving them 1 more hour and then I'm calling again. I cannot go thru a weekend just guessing that i'm on the right med.

And I will definitely follow up with my urologist as far as making sure I've got the right tx and that the infection gets wiped out.

Again, I appreciate your feedback.
