Any suggestions to prevent this? confused

Diagnosis:  UCTD (lupus) 2006; Raynauds 2006; Sjogren's 2006; lupus symptoms began 2003; UC 2008; CFS 1991; Mono 1985
Meds:  Plaquenil 400mg; Prednisone 5-10mg; Tramadol 100mg 3-4x daily; Doxepin 25-50mg; Prevacid; Evoxac; Steriod Cream and Mouth Rinse for tongue and mouth ulcers; Hydrocodone 5/500 prn for severe pain; Artificial tears

Welcome to the forum.  Sorry to hear that you're suffering so much with this unknown issue.  I'm dealing with some sort of tendon, ligament thing with my feet right now. I also have bicep tendonitis in my right shoulder.  It's weird.  It all came on so suddenly.  I've only been dealing with all this for about 8 months now.  I wish I had some insight to share with you.  I'm perplexed myself!
I did have an EMG done.  It's a test that measures if there is any nerve damage going on.  You might want to schedule one of those.  My results came back normal (figures).
Your question, if it's inflammation, or nerve problems - it could be both, one or the other, or neither!  I think more investigating and testing is required.  It's frustrating isn't it.
