lupus information check list

I am looking for that check list to take to the doctors. I saw it on here when I first started and now I can't find it. It was a detailed list of everything you need to take to the doctor. I am being referred to another specialist on the 28th and I want to have all this information ready.

Does anyone know where I can find it.?

My eye site must be failing me because I can't find it. It was a printable list to take with you at each doctors visit.
