Ok, so I have been on plaquenil for 10 months now. I am feeling better but sometimes out of the blue, I have a bad flare. I am considering asking the doc for prednisone but when I have needed it before, it took from 20-30 mg to be effective for me. I get horrible insomnia on prednisone, have type 2 diabetes and I don't want to get on the roller coaster, if you know what I mean, with prednisone. Is there anything else I can do for the flares? I've heard some bad stuff about methotrexate too. Don't know what to do but I am so sick of being sick. I had surgery for colitis a year ago and thought after that, I'd be home free but apparently now I have AI issues that are unspecified with a positive ANA, sed rate and weakly positive RF, all others negative. I've been sick a lot recently too with colds and UTI's. Does this sound familiar? Should I go to my PCP or back to the rheumy? I saw him last week and was feeling so good, he sent me home with no blood work and a 6 month follow up. Friday, I started feeling like crap again with fatigue and joint pain. Any recoomendations? Thanks!

I would start by going back to the rheumy, if I were you. Make sure to mention the colds & UTI's to him as well as the current stuff.

Are you comfortable with your rheumys' knowledge of diabetes? (Not that he's the one to treat you for it, but he needs to know about medication overlaps and such).

My old rheumy put me on a dhea supplement which I think mimics prednisone but is otc. Ask your doc to see if maybe it'd be an option. Do you take cinnamon pills for your dm?

Maybe you should talk to your doctor about the 3 medications typically tried along with plaquenil. Imuran, Cellcept, and Mtx. Also, document every day on how you feel so you can see if you are having more bad days then good days. If you are having more bad then good, something definitely needs to be done.
Hugs.
