Lupus pleural effusion, anyone?

Hi, I have MCTD and Fibro and a regular poster on the fibro forum. My MCTD has been pretty much been controlled using Methotrexate the last four years. Prior to the methotrexate, I was very symptomatic, when I was on the combo of plaquenil and pred.

A year and a half ago, I had my lower lobe of my right lung removed due to Adenocarcinoma. I had been doing quite well since my surgery. I have CT scans done on a regular basis and they check for cancer recurrence.

Early December, it was discovered that I had pleural effusion of the right lung. I had a thoracentesis to have fluid drawn off the lung. Initially the pulmonologist thought I had malignant pleural effusion...the fluid that was drawn was bloody and the exudative type. If indeed it was malignant pleural effusion, the prognosis was very poor. It was determined it was not malignant, still an exudative type and was caused by Lupus. I was put on 40 mgs of prednisone. Yes, I have the moon face and all the other crazy symptoms of being on a high dose. After being drained another scan was done a month later and I'm "filling up" again. Another thoracentesis hasn't been neede yet.

Just wondering if anyone else has had this problem and if it was resolved.

Sorry no one has responded. I don't have pleural effusion, but maybe this post will alert some members who missed it earlier.

We do have several members who have had these issues, sometimes combined with pneumonia - I think they have pulmonary Drs treat it rather than the Rhuemy.

My pleural effusion is autoimmune and my pulmonologist says it is caused by my Lupus. The fluid is neither bacterial or malignant. It does make me short of breath and having it drained is uncomfortable. I'm lucky to have a group of specialists that work together as a team. It was my rheumy who referred me to my pulmonologist. My pulmonologist works with many patients who have autoimmune connective tissue diseases.

I'm hoping this issue is not long term and resolves itself soon. I'm ready to get of this pred!!! I will take Methotrexate over prednisone anyday.
