I never had any issues with taking all 400 mg of Plaquenil in the morning. Some people experience some mild stomach upset for the first few days, but generally it goes away fairly quickly.
I believe kidney issues often first present as dark, foamy urine. I think your normal primary care physician would be the first person to investigate since you are still "feeling off" since your UTI.
Lupus doesn't generally cause eye issues. However, there is a rare eye situation that can be caused by Plaquenil. Here is more information about

You should have a baseline exam with an ophthalmologist; make sure to tell them you are taking plaquenil and they will know which test to do.
