Low white blood count and neutrophils Lupus

First time on this forum - I am usually on Lyme. Yesterday I went to a rheumotologist for osteoporosis. I think we talked about everything but osteo. He thinks I have Lupus. As many of you probably know there is a overlap of lyme and lupus symptoms.

However I have chronically low WBC and neutrophils. they have been low since at least 2012 which was when I started with regular bloodwork. This is not a symptom of lyme or at least my lyme doctor thinks it is something else and isnt concerned about treating it.

The oncologist/hematologist said that this is just "me" and no cause for concern. He would be concerned if the level changed drastically.

Is low WBC a common symptom that can definitely point to Lupus and distinguish from lyme?

Typically, four or more of the following eleven criteria must be present to make a diagnosis of Systemic Lupus.

1. Malar rash: butterfly-shaped rash across cheeks and nose
2. Discoid (skin) rash: raised red patches
3. Photosensitivity: skin rash as result of unusual reaction to sunlight
4. Mouth or nose ulcers: usually painless
5. Arthritis (non-erosive) in two or more joints, along with tenderness, swelling, or effusion. With non-erosive arthritis, the bones around joints don’t get destroyed.
6. Cardio-pulmonary involvement: inflammation of the lining around the heart (pericarditis) and/or lungs (pleuritis)
7. Neurological disorder: seizures and/or psychosis
8. Renal (kidney) disorder: excessive protein in the urine, or cellular casts in the urine
9. Hematologic (blood) disorder: hemolytic anemia, low white blood cell count, or low platelet count
10. Immunologic disorder: antibodies to double stranded DNA, antibodies to Sm, or antibodies to cardiolipin
11. Antinuclear antibodies (ANA): a positive test in the absence of drugs known to induce it.

As you can see here, low WBC is one of the diagnosing symptoms of Lupus -- however, as a Lupus patient since 2000 and a long-time moderator of this Lupus board and member of several others, I have to say I don't recall anyone ever listing it as one of the reasons for their diagnosis.

Does this rheumy have a plan to treat the Lupus he's diagnosing, or is he doing any further tests to confirm his thoughts? Have the treatments from the Lyme Dr helped with any of your symptoms?

His notes say that I have Raynauds, malar rash, low WBC, fatigue, joint pain and morning stiffness. He would like to investigate lupus or similar disease.

Raynauds I have had for 25 years. It has improved with lyme treatment but recently has gotten worse. My lyme protocol has also become less aggressive so maybe this is the reason of it? Of course I didn't remember this in my appt.

Low WBC I have no idea what is causing that and if it means anything since I am always this way.

Fatigue which could be lyme. That has gotten better with lyme treatment but I am far from perfect.

My knees seem like the only pain that could be joint related. He is sending me for an xray. IMO all the rest is muscular and the malar rash are probably due to food intolerance's caused by many antibiotics. I tried to explain that to him but I obviously wasn't convincing.

Labcorp is in his office so I didn't get to see the lab order. He did take a urine sample which is something I haven't given in 20 years.

I appreciate him looking at me with a fresh set of eyes. It was something I needed to do because I have been at lyme treatment for a while. The best thing about it is that my insurance covered the visit and it was not out-of-pocket.

This was one of those appts where I want a "do-over" so I could have been more prepared for it.

A lot of my lyme symptoms have resolved but I have the nagging cognitive, sleep and pain. I have given up on sleep and pain and am working on the cognitive. All of it could be due to my leaky gut
