Typical length of time on follow-up appts. with LD?

I have found it can vary, but I wondered how much time your LD typically spends with you on follow-up appointments?

Also, how much do they charge?

I am switching dr's, but my previous llmd scheduled my 2nd apt 3 weeks apart from the first one. then, after that it was once a month... w/out blood work, i believe it cost around 200
