My insurance co. wants a physician statement like 3 months ago. 

My LDr. won't complete anyone's long-term disability forms.

My rhuemy retired from USF on 1/31- AND I had an appt. with him for like sometime in April.

I told the insurance co. that I would try to get my pain/management dr. to complete the forms.... or my Primary Care dr. - who I haven't seen in years.

I just got a letter in the mail that my Primay Care doctor has "retired".

The next time I see my pain/management dr. will be during an out patient procedure in which I will be completely "under".

This "crappola" shouldn't be this hard! rolleyes

Hey Jelainep, Who is your LLMD that won't complete disability forms? I thought you were already on disability.

Dr. C in Tampa says he only completes forms for "short-term" disability and after that he believes the patients "primary care" doctor should be completing the forms. I was kind of pizsed off at first, but then I realized that my Rhuematologist had been completing the majority of my long-term forms.... and Dr. C did complete one long-term form approx. 1 - 1/2 years ago..... and then I get the letter that my Rhuemy has retired.... my next appointment is with Dr. S for a steroid shot between C3 and C4 and C4 and C5 - the found a bone spur (I'm just lucky that waaaay) in my neck vertebrae. I have to have the steroid shot. I can't take it anymore.... PLUS, I had a every LD/fibromyalgia/chronic fatigue type blood test done approx. 1 week ago. I should have all the results within 4 weeks, and yes, this included ALL co-infections. And, I'll have all my blood levels , etc., etc. completed prior to the steroid shot. The shots may relieve the pain I have from my shoulders down through my hands and fingers if I'm lucky. I will be put under for the procedure.

Yes, I'm a Medicare broad - haha! My Medicare SSDI was effective 05/01/08.... and within 3 months my LTD insurer, Hartford, requested an Attending Physician's Statement. It has been a CIRCUS since then.... and then to get the letter today that I don't even have a Primary Care Physician. I have to laugh or I'd do nothing but cry.

I am sorry you are having all this crap going on. When my primary doctor left where I was at, they let me pick someone else who was practicing in the same group. Is this an option or did your doc work alone? I didn't like to hear about a bone spur. That sounds painful. Other than pain meds/ steriods what is the treatment? You sure must have given them a lot of blood last time you were in. By chance is one of the tests a cd57? I would like to see you above 3.

I'm sorry your going through all that crap. I'm fighting for disability right now. I hate all the paperwork. I've been denied once already..... so the fun is just begining! Yhippie!

And, now, an update: Dr. C has taken mercy on me and is completing the 2 page (whooppee!) form for my disability carrier Hartford, since both my Rheumatologist and Primary Care Physician decided to retire at the same time.

I received a lot of my latest blood test results back. Of approx. 33 tests, they couldn't run 3 of the tests because of a mistake at Quest labs, 11 of the tests are still "pending", 7 are postive and the rest are negative. So now, just shy of 9 years since this whole 'Lyme disease - freak of nature' started, I am positive for Rocky Mountain Spotted Fever (RMSF - yay! another acronym), Typhus Fever, Klebsiella pneumonia, Proteus miabilis, Citrobacter koseri IgA, Morganella motganii IgA, Hafnia alvai IgA, NKC4, Interleukin 4 (IL4), IL8, Immunobillin (IM01) .

God I hate to say I am happy for you...but I am. I am happy that Dr. C saw the light in your case! WOO HOO

I see him on the 9th, I will bring the Champagne with me!

How are you doing?

I am hanging in, caught my 2nd cold in as many months...grrrrr

Now, they are telling me...Gee did you know for the past 2.5 yrs that you have Hashi's disease? So, Dr. running a slew more tests and in 2 weeks I expect to be put on some kind of thyroid meds and considering Bioidentical Hormones, should it show needed.

Maybe, just maybe, this is the finally piece of this very large jigsaw puzzle!

No update to any of my pending test's yet, except I seem to have made a semi-BIG insurance mistake - Lyme brain induced, I'm sure of it!  I just convinced my Pain Mngmt. doctor to complete the SAME 2 page "Attending Physician Statement" as Dr. Cichon finally agreed to do(refer to above).  I have LYME DRAIN SOOOO BAD it's getting worse again.

I get my steroid shots this Thursday; between C3 and C4 and C4 and C5 - they found a bone spur.

That is much to take in. I can hear it. You are really hurting. I am grateful you are with Dr. C and he is taking care of you.

Please be careful where the steroids are concerned. You know.

I am grateful he helped you after all.

You have been through so much.

Does he have a plan together for your treatment and care??

I just stopped everything after reading your update and prayed for your every need.

For the lovely fee of $250.00 - patient responsibility.  Thank you "Glen" from Hartford. 

So I called my buddy "Glen" from Hartford.  I pretend Glen and I are on a first name basis... he doesn't.  So I said to Glen - "Glen, Hey, it's Jennifer P****.  I'm one of your Long-term disability claimaints and.... sh** man, you are not going to believe what I did, well h*** man you just might, oh well... umm, have you received an APS from Dr. C?  No?  Well, you're about to get 2.  One from Dr. C. and one from Dr. S.  Can you believe this happened to me?  Having 2 doctors retire at once?"

And then Glen responds, "Yeah it was a new one on us too."

And, then I said, "I don't think I would believe me either."
