I used Zhang for lyme for 5 months and had to go back on ABX.  I had a hidden bartonella issue which could have played a big role in that relapse.  When I tried to take Zhang's HH capsule, just in case I had bart, I could not tolerate it.  I had really bad insomnia.  After taking Levaquin for over a month, there is no problem with the HH capsule.  I sleep fine.  So maybe the Hh capsule was working before on the BLO I did not know I had.  I also could not tolerate some other herbs that cause bladder irritation, another complaint with bart.  Now, I am also taking Samento, not the whole Cowden protocol, and I seem to feel an immediate difference in that I have more body pain.  In fact the morning has been pretty miserable but  I don't know for sure what is causing it.  My LLMD told me cat's claw (not specifically samento), teasal root and andrographis are his first choices.
