order IgeneX yourself?

Good morning everyone! Several of you have mentioned getting the IgeneX test yourself and having the doctor sign for it? How do I do that? Do I talk to the doc first or what? I looked at the IgeneX website, but I couldn't find how to order it. Going to the rheumy on Thurs to get Lupus results, etc. Want to have him sign for IgeneX.

Igenex will send you the test kits, you can order them from the website or by calling their 800#. With that comes a sheet that allows you to select which test(s) that you can have them run. The lab requires a doctors signature and a diagnosis code to be completed on the form. Igenex will fax the test results to your doctors office when they are complete, there is a place on the form where you can put that information, also.
DX in 1994 - SLE, myositis, firomyalgia, sjorgens, raynaud's, elevated ANA, lymphadenopathy.
Postive IgG and IgM 2009.  Daughter and son also have Lyme.

Yes I actually emailed Igenex and they sent the test kit. I took to my primary care dr. who could draw the blood and attached my infectious disease dr. script for lyme serology to it and he got the results and I had to get htem from him. you pay them directly with your credit card.
