I don't, sorry, we are in the Niagara region of Ontario. Our daughter underwent multiple combo abx treatment with an LLMD in Buffalo for 2 years.
She was weaned from abx in April, but continues to improve using Stephen Buhner's herbal protocols. While you are waiting to find a doctor, please don't discount the helpfulness of a good naturopath that could help you with Buhner's herbs.

And here are the books I found to be most useful. They are full of good references and make a good description of the bacteria, their requirements, the cytokine (inflammation) pathways initiated by the bacteria, the herbs used for treatment, the effects of the herbs.
Healing Lyme Coinfections: Mycoplasma and bartonella
Our daughter's symptoms were mostly caused by bartonella, yours may be something else. Lyme almost never travels alone, and often the coinfections are more problematic than the lyme. If you test negative for lyme (our daughter did), please don't dismiss coinfection involvement. These can include mycoplasma, ehrlichia, bartonella, babesia, rocky mountain spotted fever, and others.
2008 - insect bite (bruise-like rash)
2010 - Dx ADHD, motor/vocal tic disorder, high functioning aspergers, motor delay, symptoms of PANS/PANDAS autoimmune neurological syndrome.
Jan 2011 – bartonella pain symptoms begin.
June 2011 - Igenex PCR positive bartonella, IND lyme. CD57 18. Positive ANA (speckled type), heterozygous A1298C MTHFR. Multiple Abx, herbal, homeopathic treatment.
April 2013 - Weaned from abx treatment. Continuing wth Buhner, Byron White, methylation/detox protocols, Terry Wahls gf/cf/sf diet. Minimizing EMF exposure
Dec 2013 – Symptoms 98% resolved. ANA titers negative.
