If you are doing much better, that's great.
Regardless, it is very likely that you stirred up the y/f overgrowth w/ the pills but didn't quite finish the job, otherwise you wouldn't still have those sx. 5 days is a little too long to be die off, IMO. You gotta hit y/f hard and for awhile to get to it all (the pills do NOT reach all the y/f so people experiencing both yeast growing and die off, which can be quite confusing.
When the powder arrives, I would SLOWLY take your introductory doses and slowly titrate up to max doses. Follow the dosing info below but start with only 1/8 t twice a day and slowly increase in a few days, and keep increasing once the sx die down. Also, if you follow the chart, it will likely max you out because it maxes out at about
2,000,000 units per day (2 1/2 t) and your Rx was probably for 1,000,000 units per day. But try increasing per the chart and in the interim, try convincing your MD to prescribe more. With your issues, you'll need to be on the powder for a few months, perhaps longer. Even though your diet isn't hugely problematic, you seem to react to the sugar or the lack of good bacteria severely, like me.
And, if you don't implement a solid no sugar, no simple carb diet, the Nystatin is fighting a losing battle....
