From what I have learned, there are both flares and toxin overloads. Being in the body that is experiencing the issues, it's hard to feel which you are dealing with, though, especially when you add in all the possible issues that a person could be dealing with. I know that you know most of this, but I need to type out a lot of this not only so I don't get lost, but so that those that are newer will hopefully understand a bit as well.
A lot of this is trial and error, even for a highly experienced LLMD, because what works for one doesn't always work for another person. Gene mutations, mold exposure and multiple tick-borne infections can all prevent us from detoxing well - not to mention things like MCAD and other co-conditions (EBV, CMV & others) a person may have.
Flares happen because of stress (many, many forms of stress!), other co-conditions flaring (like EBV), getting another virus (cold or flu) or even just emotional ups and down and "normal" business, as that can wear us down and lower our immune function - which is why most flares happen from what I can understand.
It's a complex reaction between the immune system function lowering, too many toxins (adding more stress on the body) and the course of the infection (if treatment isn't halting it) that can cause the ups and downs of symptoms.
Personally, I learned what my body felt like when I was overloaded with toxins and would raise the amount of detoxing that I did when I felt toxic. There were times when I just couldn't tell - I'd raise up the amount of detoxing that I was doing daily (making it something I was doing all day long), yet it wouldn't make a dent in how I was feeling - so at that point, I figured it was a flare, and tried to think of any signs that my EBV was flaring, or if I had been particularly busy or stressed. Sometimes you just gotta ride it out, but usually we can at least raise our detoxing(or find more effective means for our body/situation) to help ease at least some of the symptoms.
