Great post and summary of your frustrating journey.
I'm not sure I can provide many suggestions. I understand your quasi-desperation.
I went 16+ yrs undiagnosed, went through 8-9 rounds of various abx during 2013 for chronic sinusitis, bronchitis, and nothing helped the sinusitis/bronchitis. However, all of a sudden, after all those abx my Lyme, bart and babs sx that have always been there but unidentified and not severe enough to get MD's attention—just zap my quality of life, exploded in intensity. I was finally able to get a dx.
I was in pretty bad shape when I started w/ an LLMD - spent a few months dealing with severe yeast/fungal overgrowth and GI issues, very weak immune system, thyroid & adrenals/neurotransmitters and crept along w/ Byron White protocol for the 3B's. Had various responses to nearly everything but hip and knee joints got so bad I couldn't walk. 6 mos. later LLMD started me on IV Rocephin and herxing was BRUTAL for 4-6 wks, but jt pain significantly improved over time.
A-Bart triggered monthly GI attacks, one landed me in the ER so LLMD added IV vanco - and several things improved with the vanco.
It took 2 yrs to get to optimal abx doses, I've stayed on the herbals this whole time too. And I've spent several cycles getting too aggressive and paying for it with depleted adrenals, which set me back months each time.
Babs seemed to take a backseat to all of this going on. By end of Dec I was hoping to wrap up all abx and transition entirely to herbs for a "maintenance" protocol as I considered I'd reached my 80% recovered level (need to get back to work full-time and the final remaining issues were just a matter of time - albeit a long road, still).
So my LLMD & I started biofilm-busting w/ Beyond Balance BFM-1. POW. That knocked me back considerably - nearly all sx returned but not severely. I've made progress (with one exception) and feel I'm nearing that stopping point for abx again - so glad I did the biofilm busting before stopping abx.
Anyway, just wanted to share yet another perspective of combining and switching and transitioning.
Another thing I wanted to ask - have you ever seen a neurologist? Mine is Lyme-friendly (rare) and suggested that most neuro issues are caused by inflammation. Others would disagree (certainly wasn't a solution for Rikky) and I've had moderate help w/ anti-inflammatories. Just another rock to look under.
I'm in the camp that most of us really don't know what we're treating… there are the more well-known TBDs (the 3Bs) but they're learning that the host is usually a white-footed mouse, not deer. I can't imagine the stuff swimming around a rodent's blood stream… so, it's not much of a surprise that with all of these variables, and various states of health and dysbiosis and trial-and-error, that it takes some of us YEARS to heal…
As crazy and frustrating as it sounds, you just might need to give modalities more time. Certainly nothing wrong with going back to something or trying something new. I think you could also probably increase detoxing - that always helps me.
