Dr B's treatment Guidelines for lyme mentions that ice should not be used, but heat is ok to use. You may wish to read Dr B's guidelines regarding this type of treatment.
While I usually agree with Dr. B's thoughts, there are times that heat just makes matters worse because I'm dealing with inflammation, so I turn to ice. If a person with inflammation causing an issues uses heat, it usually makes things worse, whereas ice will calm the inflammation some most of the time.
